- Day Habilitation Services
Life Skills Training Center’s Day Habilitation Program supports individuals in building meaningful life skills while fostering active community involvement.
Participants develop skills related to:
- Socialization and self-advocacy
- Daily living and independence
- Community participation
- Recreation, volunteerism, and employment readiness
Our person-centered approach encourages independence, confidence, and quality of life. Each day, clients may choose from multiple structured activities, such as:
- Basketball and fitness
- Cooking and nutrition classes
- Self-advocacy lessons
- community outings
- Creative and educational activities

- Employment Services
Employment Services at Life Skills Training Center provide individuals with meaningful opportunities to learn job skills, build confidence, and earn competitive wages.
- Supported Employment
Through supported employment, individuals receive ongoing job training and supervision while working directly within LSTC-operated services such as:
- Industrial laundry
- Floor mat services
- Janitorial services
With guidance from job trainers, Participants develop:
- Time management and task completion skills
- Problem-solving abilities
- Workplace behaviors and social skills
Clients may participate in enclave groups, working in small teams that provide services to local businesses.
- Community Employment
In our Community Employment program, individuals are hired directly by local employers. LSTC job coaches provide support by:
- Matching interests and abilities with job opportunities
- Assisting with applications and interview
- Training on job-specific tasks, social skills, and workplace routines
As individuals gain independence, job coaches gradually reduce hands-on support while remaining available to ensure long-term success

These are the steps that need to be completed for placement at Life Skills Training Center.
Secure a Payment Source
Services are typically funded through Medicaid waiver programs. Application may be completed through DHS or a Managed Care Organization (MCO). Private Pay options are also available.
In the event that your loved one does not qualify for a waiver, Private pay is an option.
Contact our business office for those rates.
Obtain a Case Worker
Case workers are usually assigned through an MCO. Mental health providers can also assist.
Apply to Life Skills Training Center
Complete this application and provide with the help of your caseworker:
- One of the following: Social history, IEP, CASH, psych evaluation, or interRAI(previously) SIS
- Diagnosis
Review and Decision
Our referral team reviews all documentation and notifies applicants of acceptance or denial.
Information Collection & Interview
Following acceptance, please provide:
- Physical and Medication List
- Social Security number
- Two forms of government issued IDs (for employment services applicants)
Authorization
LSTC and your case worker obtain Medicaid authorization.

A personalized service schedule is created, and your Life Skills Journey begins.
